This happens quite frequently.. when you leave the select menu it will force a reload of your weapon.
Also occurs randomly without being in select menu.
Happens on my private server but i have heard of this happening on other servers.

I have seen this most frequently while using the M4A1, Aug AX, and AKM but i think it may also occur with other magazine fed weapons.
