You find a new belt and put it on you. The old belt has field bottle, knife sheath and gun holster. With "triangle-key" you can only switch knife sheath and gun holster directly to the new belt - but with the field bottle it doesnt work. So you first have to drop the field bottle on the ground and then you can equip it to the new belt.
There is no sense why the direct switch does work with knife sheath and gun holster but not with the field bottle - so i think thats a bug.

To reproduce try to switch directly (with triangle-key on ps4) the knife sheath, gun holster and field botte from a old belt (e.g. in your hand) to the new belt (equipped on you).
