In the recent update it made it where you are no longer able to equip a gas mask with a headstrap, if you place the had mask on then the headstrap afterwards the gas mask will despawn after a server reset.

Steps To Reproduce
While wearing headstrap equip gas mask, removes Headstrap.
Equip gas mask then headstrap, after reset gas mask is gone.

Hello BrickToothMcGee.
This is a known issue and it has been resolved for 1.11 version. The player characters should not be able to equip the mask at the same time with NVG headstrap and the fact that it is possible through a specific way is a bug that causes the item disappearance.
