This is far fetch bug I found on my server. Friend of mine send me yesterday a picture from some guy's inventory who he shot on the field. I have nothing else to give you so far and don't think I can find much more from the logs either. This might still be usable for you to know

also: there's an item on count of 0 in there but that's known bug?
