I was in the guard tower on the North Eastern corner of the NW airfield's abandoned military base. I clicked my scroll wheel to descend the ladder to the ground and instead of turning around and descending the ladder the character jumped to the ground. There was a brief flash where the ladder descending animation kicked in after the fall, but it was too late and it registered as a death anyway.
