Today I had a similar issue.
I was swimming in a group of 5 people from prison island to the coast.
10 seconds into swimming there were some status messages. I thought these were the standard messages like, i am damped etc. Then i realized a red message saying: i am drowning. I think it repeated 10 times. Tried to swim back to the island. After that got two shocks and i died. I think the moment before the red message appeared there were also a message like "i cant breath" or something, not sure.
My char was healthy, hydrated and energized. I was definitely not shot. The four other guys made it to the coast. One hour before that i swam from the coast to the island without any problem.
