If you are carrying an item in your hands (gun, weapon, food, tool etc), and you have no space on your back (and perhaps no-space in backpack, though I have not tested that), and you try to perform any action (climbing, eating, drinking, bandaging, crafting, etc) then the object you are carrying will disappear from your hands, and teleport back to the location that you last dropped it to the ground.
