I have noticed that issue along with melee weapons.
Basically, when zombies jump/hit/etc. any action they do can sometimes trigger multiple times (I get hit by a zombie twice, and even 3 times once). When the strike happens you'll hear a 'smash' sound while twice it sounds like a rapid 'smash-smash' sound.
I think this might be one of the issues why you lose your gear quality so quickly, and why your limbs come in pain.

Steps To Reproduce

Ask a zombie to attack you.
Run around him; screw around him; do whatever;
Wait for the double strike to happen.

Additional Information

This is very dangerous since it has a chance to actually kill you in a strike (since there were many issues reported regarding zombie strength).

In fact, I detected the same issue with axes and some other melee weapons.
You have a chance to strike 2 or 3 times with 1 single hit. Guess it triggers all events while having the axe swinged.
