My Avatar randomly throws the item in his hand when another action is attempted... ig stand up/crouch/sprint

The thrown item vanishes and can not be retrieved

Other times I am stuck in the pre-throw animation and must log out and log back in (and wait 70+ seconds to rejoin server)

This has only occurred in the First Person view. At times I will also 'pop' back up if I try to crouch with a weapon in my hands in First Person. If I switch to Third Person or take the weapon out of my hands I crouch fine.
