If eat food/drink soda (or smth removing from inventory after use) from the "floor" (not from inventory), slot in inventory will filled with invisible item, what cant be removed or swapped with another thing. Fixing with removing then dressing equipment with this invisible item in slot.

If you relog you should see the item with 0% in your inventory.
Every time this happened to me it was on a laggy server.
Does not happen in 0.47 to me though

is a long bug since patch 036 and higher ingame never fixed
