Start Showcase: Infantry. Sometimes the Doc will be killed by enemy grenade launchers right as he runs up to the wounded guy though you can't kill him yourself as that gets you court-martialed.
I don't know if this is more likely to happen at a higher difficulty but a pure guess would be that elite enemies and novice teammates would make it happen the most likely.
