Usually happens in prone positions when walking off of stairs or elevated platforms. Happens often when under attack from zombie. If you are falling from glitch and have a cut or wound and try to log off. When you log back in, you will start over fresh. If you get yourself knocked out by a zombie, this is your best chance of being able to save your items. But trying to navigate this in game is very frustrating and I hope its fixed soon.
