While in inventory screen, if you have plate carrier with pouches, it is impossible to take or use things out of it. Same is with assault vest that has a butt pack, you can't do anything with it. And again same issue is with improvised belt, you can't combine it with anything while it's attached to belt. You can interact with items in them only if you're not wearing them.
