The visual cursor is out of sync with what the server thinks is selected. If you interact with the item in the first shoulder slot, sometimes the game responds by interacting with the item in your face slot. So for example, if you meant to equip your gun the game will instead put your ski mask into your hands. It has been this way since the 2nd shoulder slot was updated to allow two guns.
