You take a pistol, fully loaded and with chambered round. Shoot one time. Then do tactical reload, open the menu, grab the old mag and try to fill that one bullet. And you cannot get to grab anything on the floor you get your whole inventory bugged. The animation of reloading the mag will start, but won't reload it at all.
