This has happened to a few of my characters already. I will be traveling by myself, sitting by a fire, or any other mundane task with no present threat when all of a sudden, "You Died". I have noticed that right before this happens, there are a few game glitches preceding the drop to zero flashing health, and immediate death. Doors will no longer open or close, nor will I have the prompt of "Press Square to Open". Overall won't be able to interact with objects in the world (Excluding Inventory interactions, I am still able to drop and pick up items while in the inventory menu) before my character initiates insta-death.
