If you have any loose rounds or magazines in your plate carrier pouches, pressing the reload button will not reload any weapons using that ammo. You also can not reload magazines using loose rounds in your pouches - you have to drop them or switch them to different clothing to fill mags with them.
It seems like the game doesn't register you having anything in your plate carrier pouches
It happens every time.
