Played around with the bug a little more and although it seems to be random when it’ll start after reloading. Only way to avoid it is to reload from in your inventory by pressing B over the mag. Took me a while to figure out what was the cause. Once the bug started, you pretty much have to stop playing for the day unless you’re comfortable with only melee attacks.
As for the phantom firing. Seems to be random but from what I can tell every time you reload or even try to but unable due to the reloading bug. Your gun makes a gun shot noise. Even if you don’t have a round in the chamber or a mag for your gun.
