When you log in and out your character remains for about 15 seconds. Its not to rare for people to stalk you and just happen to kill you before you logoff. Same goes as logging in your guy will stand there for a good few seconds so if your not ina safe spot its a free kill and it wont tell you you died it will just spawn a new guy.
